SDMS W. Frederick Sample Student Excellence Award Application
The W. Frederick Sample Student Excellence Award was established in memory of Dr. Frederick Sample to acknowledge outstanding achievement in the field of diagnostic medical sonography and to encourage students to submit original research and literature review papers for publication in the Journal of Diagnostic Medical Sonography (JDMS).
Application Deadline
: May 31
Eligibility
The author must be current members of the SDMS in the Student category at the time of submission.
The Program Director must be a current member of the SDMS in good standing at the time of entry submission.
Instructions
To be eligible for the W. Frederick Sample Student Excellence Award, please fill in the requested information as completely and accurately as possible. If you have not already done so, upload your submission files after completing this application. It is your responsibility to ensure the application is completed fully. Failure to follow instructions, complete the application, or submit required files by the deadline will disqualify your application.
Required Submission Documentation
All manuscript paper files must be in Microsoft Word format and all digital image files should be in either .jpg, .jpeg, or .png format.
Electronic original copy of the complete manuscript paper submission (author identification provided on the title page only).
Electronic evaluation copy of the complete manuscript paper submission (no author identification).
Electronic copies of all illustrations/images (included in each copy type, if applicable).
Request for permission to reprint material for scholarly purposes form (if applicable).
You must upload your file through the link provided in the Instructions & Submission Deadline section of the Sample Paper Competition web page of the SDMS website OR you may access the upload link directly

Notification
All applicants will be notified via email by mid-June of submission status.
